What is the Patient Portal?

 

The patient portal is a secure online website that gives patients convenient 24‐hour access to personal health information from anywhere with an Internet connection.

Using a secure username and password, parents can view their child’s private health information, keep track of appointments, request refills, receive periodic updates and reminders and much more.

Please keep in mind that the Patient Portal is for NON‐URGENT communication only! If you have an urgent matter, please contact our office at (631) 231‐5070.

Get Started Today

  • Activate your child's Patient Portal by filling up and submitting the Patient Portal Activation form at the end of the page.
  • After your child’s Patient Portal is activated, an e‐mail will be sent to you with a Username and Password. Click on the link to the portal. Using the information in the e‐mail, place your Username and Password in the correct field and click Login.
  • You will be asked to provide your child’s date of birth and phone number. Once the data is entered, click submit
  • The portal will prompt you to choose a new password. As another layer of security, you will be asked to choose a security question and answer. Once finished click submit.
  • An eCW and Practice consent form will appear for your review. Click Next on the Consent form . On the Practice Consent form, check the box next to “I have read the consent form and the above information, then click on Agree.
  • You will be taken to our home screen and you are now ready to use the portal.

FAQs

  • Where does my health information in the portal come from?
    All of the information in the portal comes from our Electronic Health Records system . This ensures that you have access to the most accurate information possible.
  • Is my information safe?
    Yes. The Patient portal uses Secure Sockets Layer (SSL) based communication over the internet for setting up authenticated and encrypted communication between clients and servers. SSL, is the standard security technology for creating an encrypted link between a web server and a browser. This ensures that all data passed between the web server and browser remains private and integral.
  • How do I enroll?
    To gain access to the Patient Portal, you must provide us with a secure E‐mail. You will then receive a username and password. Follow the instructions in the email to complete registration.
  • What do I do if I forget my Password/Username?
    Click the link that says "Can’t access your account?" below the login. Select "Forgot Username" or "Forgot Password" and follow the directions. You will then receive an email from "Brentwood Pediatric & Adol. Assoc.." It could take up to an hour to receive it. Once you get the email, please follow the directions to retrieve your username or reset your password. If you are not able to log in, please contact our office at (631) 231‐5070.
